This program is to discuss creating of a scholarship in the name of Pattie Dawn Moon. The suggestion has been made but there is rarely time to discuss this at our meetings. So please join us at this meeting.

NOTE: Students in CDTA Agility classes must

fill out the LIABILITY WAIVER form below at the beginning of each calendar year.

Any students joining a class or coming for drop-in must also sign this form the first time you attend in a calendar year.

Instructors running dogs and those coming only for Open Practice must also complete the form.
